Parents,
Just a reminder for those in camp this week that we require a screening form for your child. Please find below the screening form which can be printed out and signed. Even if you have filled one in for previous camps, we ask that you fill in a new one with up to date information to ensure no recent symptoms/ travel abroad in the previous 14 days/ close contact with a confirmed case of Covid in the last 14 days.
We will also be asking for a form for Junior Championships so please print out an extra form if your child/children are participating in the Junior Championships next week.
